24 HRS DOMICILIARY AND OUTREACH:

Care success domiciliary and Outreach services provides specialist support to adults with Learning disability and autism in their own home. We also provide community-based services such as short breaks and respite.

We are committed to providing evidence-based, safe, effective care and support that is person centred and delivers clear outcomes.

Delivering the above can only be achieved by working in partnership with both our clients and other stakeholders. Some stakeholders e.g. social services or other professionals may be known to clients prior to our working with them. Some they will meet as we work with them, introducing them to different elements and parts of the local community with which they may not be familiar e.g. local church groups, exercise groups, arts, and craft groups or even night/day school educational activities.

For Care Success Services, Person centred care in the community is about working with individuals using our service to find what works for and with them, and meets their need for a healthy, lifestyle benefit and enjoyment. We achieve this by providing:

  • Specialist assessment and support for individuals using our service, who may have complex and challenging needs. This provides us with detailed information on what their needs are, so that a robust support plan and risk management plan can be completed and a positive behaviour plan (PBS).
  • Support to reduce and overcome the health inequalities that can be experienced by people with learning disabilities by advocating for reasonable adjustments on their behalf when we support them to access community facilities such as GP surgery, hospital appointments, and using other community services, facilities, and leisure centres.

Our support staff are trained to carry out risk assessments and manage risks resulting from the complex and challenging needs of individuals who use our service. We also provide emotional support in form of a befriending approach, for individuals that have mild to moderate learning disability. We provide assessments on activities of daily living skills (ADL), Road safety, cooking, and budgeting skills as part of support provided for those individuals who use our services. Updates are regularly shared with social worker team and families of individuals using our service. We also liaise with local specialist schools for those in education, as part of a holistic approach to supporting the individual and their family.
